Navigating services can be challenging. Access & Support Service is here to bridge that gap, offering short-term assistance to older adults, individuals with disabilities, and their carers who want to continue living independently at home. We understand the unique needs and circumstances of each person and are dedicated to providing guidance and support to help you access the services necessary for your well-being.

Our service is designed to empower you by helping you understand the available services, guiding you through application processes, and ensuring you feel confident in the choices you make. Best of all, our support is confidential and entirely free.

Our service is specifically tailored for individuals who may face challenges in accessing necessary services. Whether you are an older adult or a young individual with special needs, we are here to support you and/or your carers in navigating the complexities of service access.

Our compassionate Access & Support workers are committed to making your life easier by:

Communication Assistance:

Assisting you in contacting My Aged Care for older persons.

Application support:

Simplifying how services work and the application process.

Facilitation:

Facilitating connections with assessment services for the HACC Program for Younger People.

Expolration:

Exploring available services that fit your needs and lifestyle and helping you prepare for assessments

Guidance:

Providing guidance on accessing the National Disability Service if eligible.

Coordination:

Coordinating with interpreters to ensure clear communication and understanding of your options.

Service areas

Our services are available to residents within the six Local Government Areas of Gippsland: Baw Baw, Latrobe, South Gippsland, Bass Coast, Wellington, and East Gippsland.

Service days and times

Monday to Friday, 9 AM to 5 PM
